To meet tryout requirements, students must submit a VHSL PHYSICAL FORM and parents will need to complete the ONLINE REGISTRATION process for each participating child. Once registered, the student will need a TRYOUT CARD to turn in to the head coach each season. This card is submitted to the coach on the first day of tryouts. The Tryout Card distribution process will be shared via News You Choose & social media prior to the start of each season.
To obtain a TRYOUT CARD, the following requirements must be completed:
The VHSL form MUST say “Revised January 2021” in the top right corner of pages 1-4. No other versions of this document will be accepted. Pages 1, 2, and 4 of the form are to be filled out and signed by the athlete and a parent/guardian. Page 3 must be filled out by a doctor, with the exam signed and dated on or after May 1, 2024 to be eligible for this school year (2024-25). This must be turned in once per year to the Activities Office or the Athletic Training staff – it is good for any season or Green/Yellow day sessions during the school year (valid through June 30, 2025). If you would like to retain the original copy of the physical form, please email a single PDF document including all 4 pages and any additional clearance paperwork to LBSSActivities@fcps.edu.
Like the physical, the athletic registration must be completed yearly for any sport the student is interested in participating with. Instructions for online registration can be found on the registration homepage.

- Concussion Education Program
The athlete AND one parent/guardian must review the online Concussion Education presentation, which is part of the online registration process mentioned above. This must be completed once per year.
This information is now collected via the registration process. Please make sure this information is complete and accurate/updated, as it will be shared with EMS in the event of an emergency.
***All freshmen are academically eligible for sports in the fall and winter. High School students must pass no less than five credits the previous semester to remain eligible each season.
All required forms, contact information for coaches, tryout information (times & locations), and team schedules will be made available via the LB Athletics website https://www.lakebraddockbruins.com.
